Anemia and Your Oral Health

One of the most common clues that you might have anemia is found in your smile. If you’ve noticed that your gums are suddenly paler than they used to be, it could be a sign that you’re anemic. What does this mean for your general health—and your oral health?
There are several types of anemia, which have can be caused by different medical conditions and which can cause different symptoms. The most common form of anemia, and one of the most easily treated, is iron-deficiency anemia.
Iron is essential to our health. Why? It’s all about the red blood cells. First, some biology.
Every cell in your body needs energy to live and to function. Cells get this energy from the foods we eat, which are broken down into a simple sugar called glucose. Glucose molecules are transported by our blood cells to reach all parts of the body. But that’s only the first step.
Energy is released into the cells when the chemical bonds holding glucose molecules together are broken. And this requires oxygen. Once inside our cells, glucose and oxygen react, breaking the chemical bonds of the glucose molecules, reconfiguring their atoms to create carbon dioxide and water byproducts—and releasing energy into a form your cells can use.
So, how do we move this oxygen from our lungs to our cells to start the process? That would be our red blood cells again. But this process can’t happen without adequate iron. More biology!
Red blood cells contain a protein called hemoglobin, and hemoglobin contains iron. In fact, well over half of the iron in our body is found in our red bloods cells. And here’s the key to iron’s importance: iron attracts oxygen molecules.
Hemoglobin picks up oxygen in our lungs and delivers it throughout the body, where it becomes glucose’s essential partner in fueling our cells. Without enough iron, your body won’t be able to create the red blood cells it needs so that all of your cells, tissues, and organs can get the oxygen they need to function efficiently.
When might you suspect that you’re anemic? Your gums, the lining of the mouth, the inside of your lower eyelids, and your skin can take on a paler hue—the classic sign of anemia. But other symptoms which you might not necessarily associate with anemia are also common, including:
- Fatigue
- Heart palpitations
- Cold hands and feet
- Dizziness
- Irritability
- Difficulty Concentrating
- Pica (the desire to eat non-food items)
And because anemia affects the whole body, your oral health isn’t immune. It’s not just a matter of pale gums. Iron deficiencies can cause:
- Sores and ulcers in the mouth
- Cracks on the sides of the mouth
- Pain, redness, and/or swelling of the tongue
- Loss of papillae (those little bumps on your tongue which hold your taste buds)
- Tooth damage caused by pica
- Dry mouth, which increases your risk for cavities, gum disease, oral infections, and bad breath
Iron-deficiency anemia can have several causes, among them insufficient iron in the diet, medical conditions which cause bleeding (such as ulcers), heavy menstruation, pregnancy, or growth spurts in children and teenagers. Treatment will depend on the cause of your anemia. It might be as simple as a change in diet or an iron supplement, or it might require further treatment options. Do consult with Drs. Rottschalk, Acker, and Froidcoeur before taking iron supplements, as too much iron causes iron toxicity, which can be very dangerous.

Morning Sickness and Your Oral Health

Whether it’s your first child or you’re an experienced mom, pregnancy is an exciting time—which you’ll be able to enjoy much more once the days of morning sickness are over.
Morning sickness affects most pregnant women, and can present in any number of ways. It might take the form of mild nausea, or cause frequent vomiting. It often occurs in the morning hours, but can take place at any time of day. It’s usually over with by the second trimester, or it can last weeks longer. Because of this wide variety of symptoms, morning sickness is more accurately referred to as NVP, the nausea and vomiting of pregnancy.
Your doctor can recommend safe and effective ways to deal with NVP, especially when symptoms are more severe. We have some recommendations to minimize the effect of morning sickness on your dental health.
Wait, dental health? That’s right. Besides its more obvious unpleasant consequences, NVP can cause problems for your teeth and gums. We have a few suggestions for getting you through the weeks of morning sickness with easy and effective strategies to protect your oral health.
Prevent Acid Damage
Vomiting and heartburn with reflux expose the teeth to stomach acids. Tooth enamel is the strongest substance in the body, but it’s not safe from acid attacks.
Normally, the pH balance in our mouths is neutral, around a 7 on a scale of 0 to 14. When acidic conditions lower the oral pH to 5.5 or below, the minerals in enamel begin to dissolve, leading to sensitivity and, eventually, cavities. The pH of our stomach acid ranges from 1.5 to 2.5. This is great for digesting food, but not great at all for enamel and sensitive gum tissue.
Whenever we vomit, the immediate (and understandable!) impulse is to reach for our toothbrushes. But this is an impulse we should resist. Because acids weaken tooth enamel, scouring the teeth with a toothbrush right away can cause more erosive damage. Instead,
- Wait half an hour or more before brushing to give your enamel time to recover its strength.
- In the meantime, rinse with water and perhaps a bit of toothpaste or diluted, doctor-approved mouthwash.
Keep Hydrated
Another consequence of frequent vomiting is dehydration. It’s always important to keep our bodies properly hydrated, and this is particularly true during pregnancy.
Hydration is also essential for oral health. Saliva helps wash away the bacteria and food particles which cause cavities and irritate the gums—especially important now, since pregnancy hormones can leave you more vulnerable to gingivitis and other forms of gum disease.
- Drink water as recommended by your doctor. It’s healthy, it hydrates, and it helps prevent cavities and gum disease. Win/win/win.
- Whichever fluids are your favorites, drinking them in measured amounts through the day will be more effective and more appetizing than drinking large amounts once or twice a day to catch up.
- You have a busy life, so make things easy on yourself. If you need to hydrate more, keep track of your hydration goals with an app or a phone notification. There are even “smart bottles” to track your intake. Or go old school, and fill a bottle marked with timed measurements to remind you to hydrate through your day.
When The Very Idea of Brushing Is Nauseating . . .
When just the thought of brushing is unappetizing, we have suggestions which might help.
- Postpone brushing until later in the day when you feel better. Rinse with water first thing in the morning instead.
- Trade in your regular toothbrush for one with a smaller brush head. This can help prevent the gag reflex.
- Brush slowly, using small strokes.
- If brushing the back teeth is particularly triggering, try deep breathing or sitting down to brush.

Tooth Sensitivity

Most of us know that unpleasant feeling. You’re happily enjoying an icy milkshake with lunch or having a piping hot cappuccino break, and suddenly an innocent binge turns to cringe—your tooth sensitivity has crossed another item off the menu. If heat, cold, even the act of brushing cause you tooth discomfort, we have solutions for you. Let’s look at some of the common reasons for tooth sensitivity and how to prevent it.
- Bad Brushing Habits
There can be too much of a good thing! Aggressive brushing, especially with firm toothbrushes, can damage enamel. When the dentin underneath is exposed, heat and cold can reach the sensitive inner tooth and trigger discomfort.
Go easy on yourself: Switch to a soft bristled or electric toothbrush and brush thoroughly but gently. It’s often suggested that we massage rather than scrub. We can also recommend toothpastes that can help reduce sensitivity.
- Gum Disease
As gum disease progresses, the gums start to pull away from the teeth and expose their roots. Even though the roots are protected by cementum, a covering a lot like enamel, they are more sensitive to heat and cold. And if you are an aggressive brusher, the cementum is more easily damaged than enamel, leaving the sensitive roots even less protected.
Prevention is the answer: Brushing and flossing help prevent gum disease, as do your regular exams and cleanings. If you are suffering from more advanced gum disease (periodontitis), Drs. Rottschalk, Acker, and Froidcoeur can suggest advanced treatment to protect your roots, such as deep cleanings and gum grafts. Don’t let periodontitis go untreated, as it can lead to infection and even tooth loss.
- Your Diet
If your enamel is already compromised, sugary foods and acidic foods can cause tooth discomfort.
Be patient: Until your sensitivity problem is under control, avoiding these types of foods will help. On the other hand, sugary and acidic foods aren’t the ideal dental diet, so cutting back is not a bad idea.
- Your Dental Products
Mouthwash, whitening toothpastes and home whitening products contain substances such as alcohol or bleaching agents that can cause sensitivity in some users.
Make some changes to your shopping list: Choose products without alcohol or whiteners. Talk to us about gentler alternatives for mouthwashes and whitening.
- Tooth Injuries
If you have a cavity, a broken filling, a cracked tooth, or some other injury to the tooth, sensitivity can be a warning sign.
